Eyes are the portraits of affection and love, but the endearments provided by nature to these parts of the human body sometimes succumb to regrettable events. Retinal Detachment (RD), Corneal Ulceration, Trachoma, Vit-A deficiency, Pituitary Adenoma and Visual Aura are some of the conditions that can lead to vision loss. Retinal Detachment is defined as the separation of the sensory retina from the Retinal Pigment Epithelium by Subretinal Fluid (SRF). The main types of RD are Rhegmatogenous and Non-Rhegmatogenous RD. Rhegmatogenous RD is caused by a full thickness break in the sensory Retina while non-Rhegmatogenous RD is either Tractional or Exudative in nature. Tractional RD occurs due to pull on sensory retina by contracting vitreoretinal membrane while exudative RD is caused by the accumulation of exudate or serous fluid in Sub Retinal Space.
\nFloaters, flashing light, fall of vision, and field defects are the initial symptoms of Rhegmatogenous RD while signs include decreased visual acuity, confrontation test, relative afferent pupillary defect, IOP, Mild Anterior Uveitis and +ve Shaffer\u2019s sign.
\nSimilarly, A Corneal Ulcer is an open sore in the outer layer of the cornea that is also one of the reasons for visual impairment. It is most often caused by bacterial infection.In an identical scenario, Vit-A deficiency may also lead to optical disorder. Furthermore, Diabetic ophthalmopathy, Hypertensive Arteri-venous nicking and pituitary adenoma are some of those conditions that can mimic blindness. Migraine with brainstem aura can also cause sightlessness and visual disturbances throughout both visual fields and are accompanied or followed by dysarthria, disequilibrium, tinnitus, and peri-oral plus distal paresthesia. It is also sometimes followed by transient loss or impairment of consciousness or by confusion. This whole scenario in the final stages gives rise to throbbing headache, often with nausea and vomiting.1
\nDuring my clinical practice at Type-D Hospital, Tehsil Banda Daud Shah, District Karak, I encountered a case who suffered from visual aura. He was an owner of a medium sized restaurant, and endured agony from a sudden loss of vision along with headache. All his investigations including fundoscopy and MRI brain turned out to be normal. His eyesight returned to normal after a lapse of two days.
\nOn verbal inquiry, He gave a history of extreme depression. He was sent back home and was advised to consult a neurologist and psychiatrist at the same time. Thus, Psychiatrists, pathophysiologists and neurologists may make quantum leaps to understand the true nature of physiological and pathological association between depression and visual aura.
\nIn the similar chain of events, this writer used Nigella Sativa (Kalonji) oil in the treatment of Diabetic ophthalmopathy in one case for a decade. The results of that diabetic patient were promising. On 2nd March, 2022, the journal of Frontiers in Cellular Neuroscience published a research study, in which Thymoquinone was believed to be the main compound responsible for curative effects in Age Related Macular Degeneration (AMD) and Glaucoma. Thus, it is suggested to explore the true nature of Kalonji oil in the curative arena of ophthalmology and for final FDA approval.
\nThe FDA approved Luxturna to treat Leber Congenital Amaurosis (LCA) in the same sequence of circumstances in 2017. Perhaps, the same scenario can also be applied to Nigella Sativa oil.
\nRetinal Dystrophies are a group of degenerative diseases of the retina with marked clinical and genetic heterogeneity that can also mimic in the form of psychological and psychiatric implications. Psychiatric medications like Aripiprazole,Valproic acidand Fluoxetine, which are normally prescribed for major depression or bipolar disorder, as well as other mental disorders, may provide therapeutic benefits for patients with certain refractory retinal diseases.
\n2Furthermore, talk therapy can also be used as a tool to reduce the sufferings of these patients. In short, Psychologists and Psychiatrists of the planet must come forward to ferret out an organic whole of curative amphitheatre for taking out the downtrodden humanity with visual loss from the quagmire of anxiety and depression.<\/p>\n
